This form, which is designated VSR 1 and is also included as Appendix III-E to FOM Chapter III - Inspection Procedures, is used to obtain consent from employees for CSHOs to conduct video and/or audio recording during employee interviews and other inspection activities and releases the NCDOL to use the recordings for activities subsequent to the inspection (e.g., court proceedings).
