This form provides chain of custody verification for evidence or property obtained during the course of an OSH investigation. The carbon copy form located in the district field office is the preferred version of this form to use.
When a carbon copy form is unavailable, the electronic document is to be completed and printed for signatures and a copy of the completed form with signatures must be provided to the employer before leaving the work site.
